New Dental Patient Information Chicago
Need-to-Know Information For Your Upcoming Visit
Welcome to Smile 312 General Cosmetic Implant Dentistry! Dr. Harvey Jay Mahler and our team are eager to welcome you and your family to our dental office located in the Chicago Loop area. Delivering state-of-the-art care and service that focuses on your whole-body health, our 100% digital workflow and innovative technologies create a forward-thinking and more comfortable approach to dental care. Before arriving for your first visit, we want you to be well prepared, which is why you are encouraged to review the details provided below and call us if you have any questions.
Your First Visit
When calling to schedule an appointment, our team will send your new patient paperwork electronically so that you can fill the forms out on your phone before your visit. Once in our office, we will take digital panoramic X-rays so that our team can easily see all of your teeth in one image. Then, using an intraoral camera, we will show you the details of your teeth before your examination.
With the help of our dental assistant, Dr. Mahler will perform a thorough head and neck oral cancer exam as well as a TMJ, periodontal, and tooth exam. This will be followed up with a professional teeth cleaning. You will then receive an email that includes the X-ray taken in-office as well as a copy of our conversation and recommendations.
Dental Patient Forms
When calling our office, you can expect us to provide the necessary patient paperwork electronically so that you do not have to waste additional time sitting in the dentist’s office. You can also download the forms by clicking the links below. This helps to speed up the process and ensures you see Dr. Mahler on time.
At Smile 312 General Cosmetic Implant Dentistry, we are in-network with Delta Dental, Cigna, and BlueCross BlueShield. You can expect that our team will help to maximize your available benefits so that you can save on the services you receive. We also accept cash, check, Mastercard, Visa, Discover, and American Express.
When dental insurance will not cover the cost, or you are preparing for multiple treatments, you can enroll in a third-party payment plan with CareCredit Financing. Their low or no-interest plans make it easy to break up the cost of your care into monthly installments that match your budget. Also, there are no surprise fees.
